All full-time Concord Hospital employees are elligible for dental insurance as of the first of the month following 45 days completion of continuous, full-time service. Individuals with eligible dependents may elect to include them in coverage. The following summarizes the Core Dental benefit.
Diagnostic:
- Evaluations - twice in a calendar year X-rays (Complete series or panoramic film) once in a 3-year period
- Bitewing x-rays once in a 12-month period
- X-rays of individual teeth as necessary
Preventive:
- Cleanings - twice in a calendar year
- Fluoride once in a 12- month period to age 19
- Space maintainers to age 16
- Sealant application to permanent molars, once in a lifetime per tooth, for children to age 15
Diagnostic and preventative services are covered at 100%.
Basic Restorative:
- Amalgam (silver) fillings
- Composite (white) fillings (anterior teeth only)
Oral Surgery:
- Surgical and routine extractions
Endodontics:
Periodontics:
- Periodontal cleaning (maintenance procedures)
Only two cleaning are covered in a calendar year; this can be routine (Coverage A) or Periodontal (Coverage B), but not both.
- Treatment of gum disease
Denture Repair:
- Repair of a removable denture to its original condition
- Emergency Palliative Treatment Covered at 60%
Major Restorative:
- Removable and fixed partial dentures (bridge)
- Complete dentures Rebase and reline (dentures)
- Crowns Onlays Implants Covered at 50%
Calendar year maximum: $1,000 per person (Coverage A, B, and C combined)
