Gail Fayre MD
Program Director and Assistant Professor of Community and Family Medicine
Dr. Fayre joined the NHDFMR faculty in July 1996 after spending two years on the family practice residency teaching staff at Erhling Berquist Hospital in Nebraska. Her clinical experience includes two years at the 39th Medical Group Hospital in Turkey. Dr. Fayre's clinical practice includes obstetrics, sports medicine and outpatient procedures. Her clinical interests include:

  • Orthopaedics
  • Sports medicine
  • Evidence-based medicine
  • Outpatient procedures

J. Bryan Bannister MD
Dr. Bannister received his medical degree from the University of Texas Medical Branch, Galveston in 1998. He completed his family medicine residency at the University of Rochester. Dr. Bannister joined the NHDFMR faculty in 2007 and brings with him six years of community health center experience.

Teri Brehio MD
Dr. Brehio joined the NHDFMR faculty in September 2004. She graduated from the University of Massachusetts Medical School and completed her residency at Concord Hospital, where she served as chief resident from 2003-2004. Dr. Brehio practices at Hillsboro-Deering Family Health and is the rotation coordinator for inpatient adult medicine and ICU. Her clinical interests include:

  • Teen care
  • Preventive medicine
  • Dermatology

Suzanne Boulter MD
Adjunct Assistant Professor of Pediatrics
Dr. Boulter is a pediatrician with 25 years experience in providing care for children in the Concord community. She joined NHDFMR in 1997 to pursue public health advocacy projects for children and teens. She practices at Capital Region Family Health Center and precepts residents in the care of children and teens. Some of the projects she has been involved in include:

  • Developing health and wellness services for teens
  • Establishing Concord Hospital's "Reach Out and Read" pediatric reading program
  • Increasing children's access to health insurance and dental services
  • Developing a health-oriented guidebook to help parents care for children from birth to 6 years
  • Preventing youth suicide
  • Promoting childhood health and wellness via a local television news program
  • Developing a home-visit program that has family practice residents providing at-home care to new mothers and babies


David Conway MD
Assistant Professor of Community and Family Medicine
Dr. Conway began his career as a registered nurse, working as a labor and delivery nurse from 1980 to 1982, before attending Albany Medical College. He completed his OB/GYN residency at Hartford Hospital in 1990. He worked in a large group private practice in Portland, Maine from 1990 to 1992, then in a rural solo practice in St. Marys, Penn. from 1993 to 2001. Dr. Conway provides OB/GYN consultations for the family practice faculty and residents in general obstetrics, gynecology and infertility as a member of the faculty. 

Mary Danca MD
Assistant Professor of Community and Family Medicine
Dr. Danca received a master's degree in human genetics in 1976 and spent several years as a genetic counselor in Portland, Ore. She then completed her medical training and was in private practice in OB/GYN for 13 years before joining NHDFMR. She coordinates the gynecology and women’s health rotation and provides surgical services and consultations in obstetrics and gynecology.

George (Skip) DeVito MD
Adjunct Assistant Professor of Pediatrics
Dr. DeVito, a primary care pediatrician, has worked in Concord for the past 20 years. He became affiliated with the NHDFMR in 1995 and joined the program as a full-time faculty member in 1999. His teaching interests include neonatology, healthcare for special needs children and their families and pediatric behavioral problems. Dr. DeVito's responsibilities include:

  • Inpatient pediatric didactics
  • Developing a community-based pediatric consultative service
  • Administering initiatives to improve pediatric care at Concord Hospital

Doug Dreffer MD
Assistant Professor of Community and Family Medicine
Dr. Dreffer joined the NHDFMR faculty in 2000. He currently serves as Medical Director of both the Capital Region Family Health Center and Hillsboro-Deering Family Health. He earned his medical degree at the Ohio State University College of Medicine and completed his residency at Concord Hospital. During his residency he served as chief resident from 1999 to 2000. Dr. Dreffer currently splits his clinical and precepting time between Hillsboro-Deering Family Health and Capital Region Family Health Center. His teaching interests include:

  • Obstetrics
  • Sports medicine
  • Geriatric home care

Beverly (BJ) Entwisle MD
Assistant Professor of Community and Family Medicine
Dr. Entwisle trained in Primary Care Internal Medicine at Boston City Hospital. She entered group practice in 1988 in Concord where she pursued an interest in geriatrics. She obtained her Certificate of Added Qualification in geriatrics in 1994. She joined the NHDFMR part-time in 1995 to teach geriatrics, while continuing her primary care practice. In 1998, she became a full-time faculty member. Dr. Entwisle teaches all aspects of adult medicine. She has developed large nursing home and home care services, which function as sites for geriatric teaching.

Dan Eubank MD
Director of Graduate Clinical Education Dartmouth Medical School Department of Community and Family Medicine
Dr. Eubank worked in a local group practice for 15 years before joining NH DPMR in 1992. He served as the medical director of the community hospice program for more than 10 years. Between 1984 and 1997, he served as a founding board member of a regional health maintenance organization. Additionally, he has developed local physician support groups and authored an article on this subject. Dr. Eubank’s clinical interests include:

  • Psychosocial aspects of primary care
  • Qualitative research
  • Organizational dynamics
  • Philosophy of medicine
  • The person in medicine

Dominic Geffken MD, MPH
Director of Preventive Medicine Residency
Dr. Geffken joined the NHDFMR faculty in July 2004 as the director of leadership preventive medicine. He graduated from the University of Vermont College of Medicine. He completed both his family practice residency and preventive medicine residency at the University of Massachusetts-Worcester. He also received his Master's in Public Health from the University of Massachusetts-Worcester in June 2004. His interests include:

  • Preventive medicine
  • Behavioral health
  • Geriatrics

Bill Gunn PhD
Assistant Professor of Community and Family Medicine
Dr. Gunn, a licensed psychologist and family therapist, joined the NHDFMR in 1997. He received his doctorate in family therapy from Virginia Polytechnical Institute and State University and has 15 years experience teaching behavioral medicine at family medicine residencies in Colorado and North Carolina. Dr.Gunn co-authored a book on models of collaboration between psychotherapists and physicians. He has collaborated with the physician faculty to create an innovative curriculum for residents. He is currently conducting a research project on screening for and addressing mental health problems in community health centers.

Joni Haley MS, LMFT
Collaborative Care Service Manager
Ms. Haley, a licensed marriage and family therapist, joined NHDFMR in May 2000. She directs outpatient behavioral health services and the behavioral health internship program at the Capital Region Family Health Center. Ms. Haley supervises the masters-level and doctoral-level interns who are members of the NHDFMR interdisciplinary psychotherapy team. She also directs the collaborative pairings between behavioral health interns and residents in the clinic.

P. Travis Harker, MD, MPH
Dr. Harker joined our faculty in December 2006. He is a graduate of our combined Family Practice/Leadership Preventive Medicine Residency Program. As part of his training, Dr. Harker earned a Master of Public Health degree from Dartmouth College and focused his preventive medicine practicum on improving care overweight and obese Capital Region Family Health Center patients. His clinical interests include:

  • Weight management
  • Quality improvement
  • Motivational interviewing
  • Full-spectrum family medicine

Alison Holmes MD, MPH
Assistant Professor of Community and Family Medicine
Dr. Holmes received her medical degree from Stanford Medical School in 2000. After completing a pediatric residency at the University of North Carolina, Chapel Hill in 2004, she continued training as a general pediatrics fellow at the University of Rochester in Rochester, New York. There she received her Master of Public Health degree in 2006. She joined the NHDFMR faculty as a pediatric hospitalist in 2006. Her interests include:

  • Breastfeeding from both public health and medical perspectives
  • Pediatric hospital medicine - developing excellent systems of care for pediatric inpatients
Tina Kenyon ACSW
Instructor of Community and Family Medicine
Ms. Kenyon joined the NHDFMR faculty in 1995. As part of a team, she coaches residents through the process of completing longitudinal scholarly projects. She also coordinates the statewide community health center network incorporating the use of the Centricity EMR. Ms Kenyon contributes to resident advising/support, curriculum development, and medical student integration. Prior to her current role, she was a member of the behavioral science faculty at Brown University for several years. She possesses an extensive background in clinical family therapy, facilitation, and coaching. Her academic interests include:

  • Curriculum design
  • Transformational learning
  • Organizational evolution
  • Community-based research

Cynthia King MD
Dr. King joined our faculty in 2006. Dr. King received her medical degree from University of Massachusetts in 2002 and entered the University of Rochester Family Medicine Residency Program. She had the opportunity to serve as chief resident from 2005-2006. Her interests include:

  • Patient and resident education
  • International and migrant health
  • Women's health

Ursula Kneissl MD
Adjunct Assistant Professor of Pediatrics
Dr. Kneissl, a pediatrician, began working with our residents while practicing in 1996. She left practice to join the NHDFMR faculty in 1999. She is primarily involved in teaching all aspects of inpatient pediatric care, including acute care and management of ill neonates. She also provides outpatient consultation on pediatric cases for area physicians and precepts at the Capital Region Family Health Center. Her involvement at Concord Hospital includes quality improvement for special medical needs children, pediatric sedation and pediatric trauma care.

Michael Madan MD
Dr. Madan joined our faculty in 2007. He received his medical degree from UMDNJ in 1993. Upon completion of his family medicine residency at Maine Medical Center, Dr. Madan served as chief resident. He served as an Assistant Program Director at Maine Medical Center for several years and was the founding director at two family health centers in New England. His clinical interests include:

Molly Rossignol DO
Osteopathic Director of Medical Education
Dr. Rossignol serves as the ODME and is responsible for overseeing the AOA accredited internship. She is responsible for developing and maintaining the osteopathic curriculum, as well as hands-on teaching of osteopathic techniques. Dr. Rossignol attended medical school at the University of New England College of Osteopathic Medicine. She completed her Family Practice Residency at Central Maine Medical Center and a fellowship in manipulative medicine at Eastern Maine Medical Center.

Gregory Thesing MD
Assistant Professor of Community and Family Medicine
Dr. Thesing joined the NHDFMR faculty in 1997 after spending two years as a staff physician at the United States Air Force Academy. Additionally, he was a clinical instructor in the Department of Family Medicine at the University of Colorado Health Services Center. Prior to his work in Colorado, he spent three years as staff family physician at Eielson Air Force Base in Alaska. Currently, he practices at Hillsboro-Deering Family Health. His teaching interests include:

  • Outpatient procedures
  • Precepting
  • Obstetrics

Angela Yerdon McLeod DO
Assistant Professor of Community and Family Medicine
Dr. Yerdon received her osteopathic medical degree from the New York College of Osteopathic Medicine in 2001. Following her family medicine residency at Concord Hospital, she completed a fellowship in Women's Health at East Carolina University in Greenville, NC. She then returned to NHDFMR to join the faculty in 2005. Currently, Dr. Yerdon McLeod is the maternal child health curriculum coordinator and has successfully introduced
Centering Pregnancy group visits into our clinic.

Her clinical interests include:

  • Family centered maternity and neonatal care
  • Women's health including gynecological procedures and prevention of chronic disease
