Discharge Plannnig
During your stay, your healthcare team reviews information with you as needed to facilitate a safe and coordinated discharge. A social worker is available to meet with you and your family to
discuss questions or concerns and provide information on available resources. Depending on your needs, the social worker also assists in planning for medical supplies, equipment, or a facility that
offers you the necessary services for the next stage of your recovery. Please inform your healthcare provider if you would like to speak with a social worker. We ask that you make arrangments to leave
the Hospital by 11 a.m. on the day of your discharge to provide an earlier transition for you. Please ask your healthcare provider for additional assistance, if needed.
Transportation Arrangements
Arrangements should be made to have family or friends assist you upon discharge. They may park in Lot A or B near the Main Entrance. If ambulance or wheelchair van transportation is medically
recommended and you have a preference, please advise your healthcare provider. If you have no preference, you are transported by the Hospital's contracted ambulance service. If you do not have
transportation arrangements, please notify your healthcare provider for assistance.
