The health information management services department at Concord Hospital handles requests for copies of Concord Hospital medical records. The department, located at 250 Pleasant Street, is open Monday through Friday from 8:00 a.m. to 4:30 p.m.

Our Pledge Regarding Your Health Information

Concord Hospital is committed to protecting your health information. We are required by law to make sure that health information that identifies you is kept private; to provide you this notice of our legal duties and privacy practices with respect to your health information; and follows the terms of this notice. We invite you to read our notice of privacy practices.

Prior to making a request with us, you should know:

  • Our policies have been designed to protect your right to privacy and to allow you to control the release of information from your record.
  • You have a right to access your own record.
  • There may be a fee for the services you request. State law regulates our fees.
  • We prioritize by type of request. Our first priority is the requests that are necessary for the continuation of medical care. It usually takes between 5 to 15 working days for us to provide the information requested, depending on the request type. We will work with you if you have special deadlines.
  • We need your request in writing so that we have a legal record of your request and to verify that you have given permission for the release of information.

In order for your request to be processed you must complete our personal health information release authorization form. The authorization file has been converted to Portable Document Format (PDF). Copies for Your Non-Concord Hospital Physician or Other Care Provider

  • Mail us your completed authorization form or a letter explaining your request and include the date of any upcoming appointment you may have, so we can make sure the copied records get there on time.
  • It usually takes 5 to 10 business days for a provider to receive the copies.
  • There is no fee for this service.

Copies for Your Personal Use

  • Mail us your completed authorization form or a letter explaining your request.
  • Include your mailing address and telephone number.
  • Usually your records will be mailed to you 10 to 15 business days from the date we receive your request.
  • There is a fee for this service and payment is expected prior to release of copies.

Copies to Pay an Insurance Claim

  • The insurance company requests what it needs from us.
  • The company sends a request to us signed by you permitting us to release the information.
  • There may be a charge to the insurance company.
  • It usually takes 5 to 10 business days.

Frequently Asked Questions about Requesting Copies of Medical Records

Do you provide copies of birth certificates?

No. The Concord City Clerk's Office provides certified copies of birth records to immediate family members for births that occurred in Concord, NH prior to 1989 as well as for births that occurred in any city or town in the State of New Hampshire after 1988.

I am an adoptee seeking information about my birth parents, who do I call for this information?

What happens to my information after it is released?

You can ask the receiver of this information how it is used and stored.

How much information do you send for each request?

We limit the information sent to the minimum necessary for the purpose stated in the request.

Can I obtain a copy of medical records other than my own?

Generally, no. There are some exceptions. You will need to provide written documentation to verify any of the instances below.

  • If you have written authorization from the patient.
  • If you are the patient's legal guardian.
  • If you have been appointed by the patient as their legal representative.
  • If the patient is under 18 and you are the parent or legal guardian.

How much will I be charged for copies of my medical record?

On August 18, 2001 a bill that amended New Hampshire law relative to charges for medical record copies became effective. In compliance with this bill (SB 42) all requesters (except for copies sent directly to licensed healthcare providers and other healthcare facilities) will be charged $15 for the first 30 pages copied and $.50 per page for each additional page over 30.
