Why do I need an insurance referral?

Depending on what type of insurance (HMO, PPO or POS) you have, some companies require an insurance referral prior to receiving treatment from a specialty office. A referral can be obtained from your primary care physician who can mail or fax the referral to our office. A referral ensures your bill is processed appropriately through the insurance company.

What process do I take, if I need an appointment?

Our office hours are 8:00 a.m. – 5:00 p.m. Monday through Friday. If you need an appointment, most of the time your primary care physician will refer you to our specialty practice. Some insurance companies require a referral prior to making an appointment. Your primary care physician can send us your medical records directly, leaving less work for you.

What should I bring to my appointment?

Please bring your health insurance card to each appointment. Your insurance and demographic information (name, address, etc) will be verified at each visit. Bring the completed paperwork we sent to you prior to your appointment, including the consent to treat form and medical history form. We appreciate your cooperation and patience with this process. Co-payments are due on the date of your appointment.

How do I get my test results?

You will be contacted verbally by telephone with any abnormal test results, and within 10 days with normal results. If you are not contacted within two weeks, please call the office.

How do I request a prescription refill?

Our office requires a 48-hour notice for prescription refills. Call our office directly to initiate a prescritption refill. You will be transferred to an automated prescription refill line. This line will ask your name, date of birth, medication, dosage and what pharmacy to send it to. To make it easier for you, have the empty pill bottle in front of you when calling our office. Mail-away pharmacy prescriptions are mailed directly to you, call the office to make these arrangements. Prescriptions will not be faxed, it is your responsibility to mail the prescription to the mail-away pharmacy. Reminder most mail-away pharmacies require you send in the prescription at least 2 weeks before you need the medication.

Prescription refills of narcotic or controlled substance medications, contact us at least 48 hours prior to the date needed and you must pick up the prescription at our office, they will not be mailed.

Why would I be scheduled to see a nurse in the office and not a provider?

Our nurse practitioners and physicians have an excellent working relationship. They work very closely as a team to provide exceptional comprehensive, and timely care to all our patients. Our nurse practitioners have the skill level, knowledge and specialty background to provide you with the best possible care.

How do I let you know if it is okay to discuss my healthcare issues with my spouse/family/friend?

Confidentiality is very important to us. In order for us to discuss your medical information with family, friend or other designees, you must complete a verbal release of information form. This gives our office permission to discuss your healthcare with someone you designated. Forms are available in the office.

What if I cannot afford to pay for my services?

Pulmonary Medicine, as a department of Concord Hospital, offers a Financial Assistance Program. If you are unable to pay for healthcare services, this will not prevent you from receiving the care you need. What insurances does Pulmonary Medicine accept?

Aetna, Anthem, Cigna, First Health/Coventry, Great West/One Health, Harvard Pilgrim, MVP, Martin’s Point Health Care, Medicaid, Medicare, PHCS, Tri-Care and United Healthcare. Co-payments are expected at time of service. Your bill will be submitted to all commercial insurance companies. If payment from the insurance company is not received within 30 days, the balance will become your responsibility. For your convenience, we accept MasterCard, Visa, Discover and American Express.

How do I get an appointment to see someone at Pulmonary Medicine regarding my sleep disorder?

Typically your primary care physician or provider will refer you to our specialty office directly for a consultation appointment with one of our specialists. Our office will then mail you a packet in the mail, including a sleep questionnaire, information on our providers and directions to the office. Complete the sleep questionnaire and mail it back to our office. Once we receive it we will contact you to set up an hour-long consultation with a specialist.

I mailed my questionnaire into the office, why I haven’t I been contacted?

Our office processes incoming questionnaires weekly. If we have not contacted you perhaps we do not have the correct telephone number for you or the information did not reach us. Call our office at (603) 224-9661 and inquire, we would be glad to assist you and set up an appointment.

When I come in for my first sleep consultation appointment, will I be staying overnight?

The answer is no. Your first appointment is with a provider in our office for a consultation. This hour long appointment is where the provider will interview you, to determine if you are a candidate for a sleep study and what the best course of treatment would be for you. If it is determined you are a candidate, our office will set up an overnight sleep study appointment at the sleep lab before you leave our office.

Why do I need to come in 15 minutes early?

We register each patient for his/her appointment to ensure address/telephone numbers are accurate, most currently insurance is on file, insurance address/ID number is accurate. Our goal is to ensure the information is up to date so claims are processed correctly and you are not billed unnecessarily. Also when we need to mail/call you we have the accurate information on file.
