Concord Hospital’s Heart Wellness Program offers a complete approach to the care of patients with heart failure. Focusing on education and patient self-management
behaviors, the program assists in the coordination of care and services to the patient during the Hospital stay and continues after discharge.
Program Highlights:
- Evaluation and consultation during Hospital stay;
- Individualized patient education during Hospital stay including: medications, diet, managing symptoms, and exercise;
- Routine telephone calls to patients by registered nurses following Hospital discharge;
- Intervention and follow-up if symptoms worsen;
- Education programs;
- Quarterly newsletter;
- Consultation with physical therapist to develop home exercise program;
- Follow-up through the Heart Failure Clinic at Concord Hospital Cardiac Associates with a nurse practitioner or cardiologist, as needed.
Concord Hospital has one of the most comprehensive programs in the State to assist patients with heart failure management. The individualized education begins in the
Hospital, but continues long after discharge. The Heart Wellness Program team includes nurses with specialized education in heart failure management, dieticians, physical
therapists, and social workers. Our goal is to work with each patient to ensure they are receiving appropriate medications based on national guidelines, and to help them
recognize symptoms and seek treatment quickly to improve long-term quality of life and avoid readmission to the Hospital.
