Concord Hospital’s Heart and Vascular Wellness Program, which is free of charge, offers a complete approach to the care of patients with heart failure. Focusing on education and patient self-management behaviors, the program assists in the coordination of care and services to the patient during the hospital stay and continues after discharge.

Procedures Performed or Problems Diagnosed and Treated:

  • Evaluation and consultation during hospital stay
  • Individualized patient education during hospital stay including: medications, diet, managing symptoms, and exercise.
  • Routine telephone calls to patients by registered nurses following hospital discharge
  • Intervention and follow-up if symptoms worsen
  • Education programs
  • Quarterly newsletter
  • Consultation with physical therapist to develop home exercise program
  • Follow-up through the Heart Failure Clinic at Cardiac Associates with a nurse practitioner or cardiologist, as needed
